How do I get started in this program?

Our Hours of Operation at Children Young Adults and Families (CYAF): 
Monday thru Friday from 8:00am to 7:00pm
 
Where to go: 
Individuals not currently receiving services with Clayton Center begin by scheduling an intake at Clayton Center’s Child and youth services ( Call 770-473-2640 and the front desk will be happy to assist you with any questions you have about opening a chart at our program ~ walk-in hours are Mondays 8 am until 10:30 am and scheduled appointments are Tuesday –Friday.  -APPT (1-844-438-2778). At the time of intake, request a referral to Children Young Adults and Families.

What to bring with them:
At the time of intake, all individuals must bring picture id, insurance information, proof of address and proof of amount and source of income.  Children and Adolescents who present without insurance will receive assistance to complete applications for Medicaid and PeachCare for Kids. If you are a foster parent bring Guardianship papers

Who uses this program (target audience)?

Child and Adolescent services treats children from age 4 to age 21, if they are still in high school.

What does this program do?

  • We provide psychiatric assessments and services.
  • Nursing assessments
  • Medication management.
  •  Individual therapy/Group therapy  
  • Family therapy and training.
  • Community support services

How are services structured(group setting, 1:1, in home, etc)?

  • Services are provided individually and in groups.
  • Community Support Services maybe provided in the home, school, community

Intensive Outpatient Anger Management

  • Develop individualized anger control plans
  • Integrate relaxation skills with cognitive focused communication skill interventions
  • Curriculum developed by SAMSHA (Substance Abuse and Mental Health Services Administration
  • Group held one time per week for 8 weeks

Weekly Anger Management

  • Develop individualized anger control plans
  • Integrate relaxation skills with cognitive focused communication skill interventions
  • Group held once a week for 6 weeks
  • Curriculum based on Cognitive Behavioral Therapy Model

Girls Self-Esteem Group

  • Designed to develop and promote self-worth
  • Group held one time per week for 6 weeks

Intensive Outpatient Addictive Disease Program

  • Designed for adolescents ages 13 to 18
  • Mission is to motivate decisions and commitments to change
  • Curriculum developed by SAMSHA (Substance Abuse and Mental Health Services Administration
  • Group held twice a week for ten weeks ( 11/2 hours per session/20 sessions)
  • Coping skills for high risk situations

ADHD Social Skills Group

  • Designed for ages 7-10
  • Opportunity to learn and practice skills to mange symptoms
  • Parents are informed of the child's progress
  • Focus placed on the child's strengths and weaknesses

Community Support Services (formerly CSI)

  • Provide in home support services for the client/family
  • Helps families find resources in the community
  • Partner with the school systems to help promote communication between the family and educators
  • Provide support and guidance in crisis situations.
  • Reinforces services and skills that the client receives in therapy.
  • Reinforces and reviews treatment goals with client.
