Addictive Disease Support Services (ADSS)

Philosophy of Care 

Addictive Disease Support Services (ADSS) are intended specifically for adults with addictive disease issues. ADSS services consist of substance abuse recovery services and supports which build on the strengths and resilience of the individual and are necessary to assist the individual in achieving recovery and wellness goals as identified in the Individualized Services Recovery Plan (ISRP). Services are provided in a multitude of settings such as, the consumer’s home, work, and other desired locations within the community.

What is the intended goal of this program?

ADSSS service activities include assistance to the individual and other identified recovery partners in the facilitation and coordination of the ISRP, to build skills to support and promote the individual’s self-articulation of personal goals and objectives.

ADSS services are provided during the traditional and non-traditional hours with flexibility to meet with individuals in the evening and weekends when necessary.

Adult Psychosocial Rehabilitation-Individual (PSR-I)

Philosophy of Care

Psychosocial Rehabilitation-Individual (PSR-I) services consist of rehabilitative skills building, the personal development of environmental and recovery supports considered essential in improving an individual’s functioning, learning skills to promote the individual’s self-access to necessary services and in creating environments that promote recovery and support the emotional and functional improvement of the individual. PSR-I provides skills support in the individual’s self-articulation of personal goals and objectives. PSR-I staff assist the individual in the development of skills to self-manage or prevent crisis situation. The primary focus is on promoting recovery, symptom reduction, increased coping skills, and achievement of the highest level of functioning in the community. Services are provided in a multitude of settings such as, the consumer’s home, school work, and other desired location within the community.   

What is the intended goal of this program?

PSR-I is provided to individuals with mild to moderate mental health impairments that can affect their optimal functioning. The goal of PSR-I is to help them maintain stability and independent in their daily living at home and within the community, as well as assist with community integration/reintegration.

Case Management

Philosophy of Care 

Case Management (CM) services consist of providing environmental support and care coordination considered essential to assist the individual with improving their functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in the Individualized Service Plan (ISP). The focus of the interventions include: assisting the individual with developing natural supports to promote community integration, identifying services needs, referring and linking to services identified on the ISP to maximize service integration and minimize services gaps and ensuring continues adequacy of the ISP to meet the individual’s ongoing and changing needs.

What is the intended goal of this program?

The goal of CM services is to decrease hospitalizations, incarcerations and homelessness. The expectation is that individual’s receiving CM will have increased housing stability, participation in employment or job related activities, community engagement and recovery maintenance.

CM services consist of four major components that cover multiple domains that impact one’s overall wellness that include medical, behavioral, wellness, social, educational, vocational, co-occurring, housing, financial, and other service needs of the individual. The four major components are Engagement and Needs Identification, Care Coordination, Referral and Linkage, and Monitoring and Follow Up.

CM services are provided during traditional and non-traditional hours with flexibility to meet with individuals in the evening and weekends when necessary.  Adults who present without insurance coverage may be assisted with application for Medicaid, Affordable Care Insurance, and other general entitlements.
