How do I get started in this program?

A referral to the Health and Wellness Management Program (HWM) is made by the intake specialist during the intake assessment.  

Where to go:

Individuals not currently receiving services with Clayton Center begin by scheduling an intake at Clayton Center’s Adult Behavioral Health Services Center, 853 Battle Creek Road, Jonesboro, GA  30236.  Clayton Center intakes are scheduled by calling our Centralized Scheduling line at 1-844-GET-APPT (1-844-438-2778). At the time of intake, request a referral to Adult Day Services.

What to bring with them:

At the time of intake, all individuals must bring picture id, insurance information, proof of address and proof of amount and source of income.
 
What do I need to bring to the appointment?

Any documentation and information that assists the medical clinician in making a diagnosis or confirm a diagnosis should be brought to the Health and Wellness Management appointment. For children this includes behavioral reports from school, report cards/progress reports, behavioral surveys/questionnaires, and prior behavioral health and or schools assessments or testing.  For all patients, hospital discharge summaries, prior assessments, medication bottles and or prescriptions, and lab work also should be brought to appointments.   For all Adults with prior diagnosed ADHD/ADD, formal psychological testing performed by a psychologist must be brought to the Health and Wellness Program initial evaluation in order for medications to be prescribed or continued. 

Who uses this program?

Adults, children and adolescents experiencing psychiatric or behavioral symptoms, whom have been diagnosed with a prior psychiatric condition or are those in need of a psychiatric evaluation.

What does this program do?

We first conduct a psychiatric assessment exploring psychiatric symptoms and psychiatric history, prior treatment, family of origin,  social and employment history, and trauma history. Treatment recommendations are based on the assessment which may include psycho-education, supportive counseling, health and wellness coaching and pharmacologic interventions/medications. Referrals to other services within Clayton Center or the community may also be suggested. 

What can I expect at a health and wellness management appointment?

Typically at each appointment, a nursing assessment occurs first which entails information gathering by the RN, LPN, and or medical technician, vital signs and education. The psychiatric assessment usually follows the nursing assessment provided by a Board Certified/Board Eligible Psychiatrist or Psychiatric Nurse Practitioner. If lab work or injection medications are ordered, the nurse will provide these services at directed times. Most lab work requires a fasting status which means no food or drink before lab work is obtained. The doctor and or nurse will provide direction on if a fasting is necessary. 

When are services provided?

Health and Wellness Management services are offered during the daily operational schedules of each program thus start/end times and days vary.  Daily services occur typically between Monday- Friday, 8am to 5pm. After-hour day and times are at the discretion of the program. 

What about medication refills? 

If a medication refill is needed, please allow 4-5 days for the prescription to mailed or called in.  Benzodiazepines will not be called in. The Food and Drug Administration mandates that stimulants cannot be called in to the pharmacy, a written paper prescription is always needed for stimulants,  only a 30 day supply can be given for stimulants and no refills can be provided for stimulant medications. 

If I have a question or concern, whom do I contact? When can I expect a phone call?

Call the program location where you receive your services.  Most calls are first given to the nurse on site to obtain more information about the issue or question. Phone calls from the physician occur after the nurse or patient advocate has attempted to address the issues. The HWM strives to return all phone calls within 24-48 hours. 

Where are services provided?

Services are offered at all of Clayton Center locations except the Administration Building.

What is the intended goal of this program?

To obtain psychiatric and or behavioral stability resulting in optimal functioning and living.
